The Patient Services Department at Jacksons Health improves access to health care and assures that patients can access health care without insurance, and regardless of insurance status. Our BIPOC owned sites offer access to quality health care for patients who don’t have insurance or don’t have enough insurance through a sliding fee discount program, with discounts available based on your current household size and income only. The program can cover you for up to 12 months—a new application is required yearly or when your household size and income changes, whichever is sooner.
Sliding Fee Scale Discount Program The Sliding Fee Discount Program assures that patients without health insurance coverage have access to all primary care services. Eligibility for the sliding fee scale discount is based upon poverty guidelines established by the federal government and are determined by family size and income.
Privately Insured Patients Patients with high-deductible health plans that require out-of-pocket expenses in excess of $2,500 annually will be extended the opportunity to apply for a sliding fee scale discount. Determination is based on documentation and verification requirements. If eligible, We will apply the sliding fee discount by charging a fee no greater than what would be charged to a patient that has no public or private insurance in a defined income category.
To apply for sliding fee services and alternate payment arrangements you will need to provide proof of household income. We will require a current valid photo I.D. and any one of the following items:
- All pay stubs from the past 30 days
- Letter from your employer or a declaration of income
- Unemployment benefit letter
