You will be asked to provide a photo identification and an e-mail address. It is important that you come to the hospital and show your ID to set up an account because your medical records will be tied to your billing. As soon as you provide us this information, an email will be sent to you that will take you through a simple, five-minute process to set up your patient portal account.
